Medical Forms and Training

I have information on when I can get medical forms for three of our six crew members.  Please respond via text to let me know when I can get a medical form from you too.

• print it out
• fill out and sign all the personal information that you know (it will repeat itself a few times)
• sign where you need to sign
• bring the form with you to the exam and tell the doctor you need his signature.
  
• tell the doctor during the exam that you will be attending a high adventure base.
  
• the doctor will sign the back page when the physical check up is done.
• save a copy for yourself and make two copies, one for me and one for Dr. Dolle
  

All leaders need Youth Protection Training (YPT) and VIRTUS (aka Safe Haven) training.  VIRTUS is the YPT for the Catholic Church and since our troop is at a Catholic Church then all leaders/18+ need that training.  That includes JS, WT, BM, and RS.
